For ophthalmologists, the clinical reality is clear. More diabetic patients are presenting with complex retinal complications that require highly precise, time-sensitive surgical intervention. As patient volumes increase and surgical demand outpaces the supply of trained specialists, efficiency, safety, and outcomes are no longer optional. They are essential.
It is against this backdrop that Alcon introduced UNITY VCS, its next-generation combined cataract and vitreoretinal surgical platform, during the Evolving Practice of Ophthalmology Middle East Conference (EPOMEC) 2025 in Dubai. The launch was not positioned as a routine product unveiling, but as a response to a mounting regional and global clinical need.

Alconโs approach to innovation is grounded in sustained investment. According to Narayanan, the company reinvests approximately 10 percent of its global revenue into research and development.
โAt any point in time, we have about 100 projects ongoing,โ he said. โThis year alone, we are launching eight to ten new products globally. UNITY VCS is one of them, and it represents a transformative change in how cataract and vitreoretinal surgeries can be performed.โ
That level of R&D investment has not gone unnoticed by clinicians. Reflecting on the pace of technological evolution, Dr. Al Messabi remarked, โTo see a company spending 10 percent of its revenue on R&D is almost unheard of in our field. But it proves itself in the quality we see. Every platform feels like the best, until the next one comes and surpasses it.โ
Precision surgery, enhanced efficiency
Diabetes-related retinal disease remains one of the most complex surgical challenges in ophthalmology. Patients often require vitreoretinal procedures to manage complications such as diabetic retinopathy, macular edema, or vitreous hemorrhage. These surgeries demand extreme precision, minimal intraocular trauma, and efficient execution.
As UNITY VCS enters the Middle East market, both industry and clinicians see strong alignment between technology capability and regional expertise.

โThe system allows surgeons to perform cataract surgery and vitreoretinal surgery on the same equipment,โ Narayanan explained. โThis is especially relevant for diabetic patients, who often develop retinal complications requiring surgical intervention.โ

Among its key features is a 30,000-speed vitreous cutter, enabling faster vitreous removal, and a multi-spot laser system, replacing traditional single-spot lasers.
โFor the first time, Alcon is introducing a multi-spot laser,โ Narayanan noted. โInstead of applying one laser spot at a time, surgeons can deliver multiple spots in a single pulse. That means faster treatment with greater precision.โ
The platform also addresses a long-standing challenge in probe design. Surgeons had previously highlighted the trade-off between smaller incisions and probe stability. UNITY VCS introduces a 27-gauge probe with the stiffness of a 25-gauge, balancing minimal invasiveness with surgical control.
โThese refinements allow surgeons to work faster and with less energy inside the eye,โ Narayanan said. โLess energy means less trauma, which directly benefits the patient.โ
Training, data, and responsible adoption
Alconโs strategy does not stop at technology deployment. Recognizing that advanced systems require equally advanced training, the company has expanded its global network of Alcon Experience Centers, including a newly opened regional educational training facility in Jeddah.
โWe invite surgeons to test our equipment, train on artificial and animal eyes, and understand how different settings impact outcomes,โ Narayanan said. โItโs about ensuring surgeons are confident before they ever use the technology on a patient.โ
He likened the process to performance training. โIf someone puts a Ferrari in front of me and I donโt know how to drive it, itโs another car. Training is what unlocks the value of the technology.โ
Data generation is another critical pillar. Alcon conducts clinical studies to quantify efficiency gains, safety improvements, and outcome consistency. โWe donโt just claim efficiency,โ Narayanan said. โWe generate data to demonstrate it. Patient safety is non-negotiable.โ
A platform with regional relevance
As UNITY VCS enters the Middle East market, both industry and clinicians see strong alignment between technology capability and regional expertise.
โThis part of the world has very qualified surgeons,โ Narayanan observed. โIntroducing a sophisticated platform here makes sense because there are skilled professionals ready to use it to its full potential.โ
For Dr. Saleh, the technology represents more than equipment. โThis platform will be in operating rooms for the next eight to ten years,โ he said. โAdopting the best technology is not promotion. It is a natural decision when it improves how we care for patients.โ
