Healthcare systems around the world have made significant progress in connecting providers and enabling the secure exchange of patient information. Yet as Health Information Exchanges (HIEs) mature, a new challenge has emerged: exchanging data is no longer enough. To unlock the full value of digital health and deliver world-class care, data must also be interpreted consistently, regardless of its source.

This is becoming one of the defining challenges of the next phase of digital health. A connected healthcare system creates access to information; a truly interoperable one ensures that information carries the same meaning wherever and whenever it is used.

For laboratory data, this means ensuring that the same clinical test carries the same meaning across every hospital, clinic and laboratory. Without this consistency, healthcare systems face difficulties in bringing together information, comparing outcomes, generating reliable population health insights and supporting trustworthy artificial intelligence applications.

Abu Dhabiโ€™s experience through Malaffi – the regionโ€™s first HIE platform that safely and securely connects public and private healthcare providers in the Emirate, demonstrates how large-scale terminology standardisation can be successfully implemented across an entire healthcare ecosystem.

Through the strategic vision and strong governance of the Department of Health โ€“ Abu Dhabi (DoH), the regulator of the healthcare sector in the Emirate, ecosystem-wide collaboration and continuous data quality management, Malaffi has achieved nearly 90% adoption of the international Logical Observation Identifiers Names and Codes (LOINC) standard for laboratory data. Beyond the percentage itself, the significance of this achievement lies in what it enables: moving from simply connecting healthcare information towards creating data that can be consistently understood and meaningfully used across the healthcare ecosystem. This cements the Department of Healthโ€™s efforts towards strengthening the Emirateโ€™s position as a global leader in digital health and providing a practical model for healthcare systems seeking to advance interoperability beyond connectivity.

Connectivity was the Foundation, standardisation is the next step.

Abu Dhabi’s healthcare ecosystem is rich and diverse, offering comprehensive care across all medical specialities, with a robust network of public and private facilities that each leverage advanced electronic medical record systems tailored to their unique operational needs. Since the establishment of Malaffi, in 2019 by DoH in partnership with M42โ€™s Abu Dhabi Health Data Services (ADHDS), the platform has enabled real-time sharing of patient health information by connecting more than 3,000 private and public hospitals and healthcare facilities across the Emirate. Malaffi also empowers clinicians with access to comprehensive, longitudinal patient records, enabling better-informed decisions, reducing redundant procedures, and ultimately improving patient outcomes in line with the Departmentโ€™s vision of a โ€˜Healthier Abu Dhabiโ€™.

But as digital health infrastructure matures, connectivity alone cannot be the end goal. The next step is ensuring that the information moving through that infrastructure can be interpreted consistently and used confidently at scale.

This achievement positions Malaffi among the few global systems/HIEs that have adopted large-scale LOINC standardisation. It enables true system-wide analytics, consistent clinical interpretation, emirate-wide public health application and the development of standards-dependent AI models with high accuracy and reliability.

Standardisation is not a one-time technical exercise

The LOINC adoption plan was carried out through a combination of structured code mapping, system integration, and continuous data quality monitoring. During which, Laboratory test catalogues from each provider were mapped to standard LOINC codes and embedded into laboratory information systems at source, ensuring that results were generated in LOINC. These standardised feeds were integrated across the HIE, supported by real-time validation rules and feedback loops to identify unmapped or incorrectly coded tests.

Quality dashboards allowed both the Health Informatics Team at Malaffi and participating providers to track coverage, consistency, and drift over time, ensuring that standardisation was an ongoing process rather than a one-off achievement, especially as new tests and panels were introduced. Additionally, DoH has access to existing monitoring tools that help the Malaffi team govern the initiative and implement the mandate in the market whenever necessary.

The lesson is important: healthcare data standardisation cannot be treated simply as a technology implementation. It requires sustained governance, ecosystem-wide collaboration and a structured approach that ensures standards evolve alongside the healthcare system.

From standardised data to better healthcare intelligence

Standardising laboratory data using LOINC primarily strengthens the digital backbone of Abu Dhabiโ€™s healthcare system without impacting clinicians’ day-to-day work. For clinicians, it reduces ambiguity when reviewing results from different laboratories and aids the understanding of trends (particularly for patients who move across providers) by ensuring that the same test consistently carries the same clinical meaning.

For patients, this improves continuity of care by enabling their laboratory history to be viewed and interpreted consistently regardless of where care is delivered. At the regulatory level, it allows the Department, through Malaffi, to reliably aggregate and analyse laboratory results across the entire population, supporting evidence-based policy making, informed decision-making and comprehensive population health initiatives that safeguard the health and wellbeing of all community members.

This is where standardisation begins to demonstrate its wider strategic value. When clinical information can be consistently interpreted across an entire healthcare ecosystem, it becomes more than a record of care delivered; it becomes a foundation for better insights, stronger population health management and the responsible use of emerging technologies.

Looking ahead: The future of data standardisation

As healthcare systems increasingly adopt artificial intelligence, predictive analytics and precision medicine, the quality of underlying clinical data will become a defining factor in their success.

Abu Dhabiโ€™s experience demonstrates that building a connected health information exchange is only the beginning. Unlocking its full value requires ensuring that every data element exchanged across the ecosystem can be consistently understood, trusted and reused.

The future of digital health will therefore depend not only on how much data healthcare systems can connect, but on how well they can govern, standardise and translate that data into meaningful intelligence.

By achieving nearly 90% LOINC adoption across the Malaffi platform, Abu Dhabi has shown how DoHโ€™s ecosystem-wide governance, continuous collaboration and internationally recognised standards can transform connected healthcare data into a strategic asset for clinicians, regulators and the future of healthcare, advancing an intelligent, proactive, and patient-first healthcare ecosystem centred around patients and communities.
